If you do not have an insurances with which Dr. Benderev is contracted (In Network),
the cash prices due at the time of service are:
Initial Consultation Cost: $150
Vasectomy Cost: $990
Only for the vasectomy procedure, a 3% convenience fee will be added at the time of the procedure for payment by credit card. The additional fee can be avoided with payment via Zelle, debit or HSA/FSA cards. Check and cash payments are not accepted.
Information for patients with
“Out Of Network” or Non-Contracted Plans
If you check with your insurance company and find that Dr. Benderev is “Out of Network”, ask if you are eligible for a partial reimbursement of your cash payment for services. It’s your responsibility and a good idea, before seeing your provider, to reach out to your insurer to check any benefits with Dr. Benderev (via the phone number on the back of your insurance card or via their website). You’ll want to ask about further actions that you might need to take to collect any “out of network” benefit payment after the claim is submitted, as well as checking on your deductible amount and restrictions.
